Miner was doing maintenance on 3' Cone/Crusher. Had removed head and was replacing it and lowered head down into bowl assembly. The head was not down on one side and the chains had been released, he stuck the two fingers of left hand into 1/2" gap, the head then released crushing the two fingers to the first joint.
EE WAS INSTALLING RETURN ROLLER ON A 3C" FIELD CONV WHEN ONE ROLLER CAME LOOSE AND HIT HER ON THE R SHOULDER & THUMB HAD TORN LIGAMENTS IN R HAND THUMB

BAR MAIN HOPPER GRIZZLY HAD SLIPPED OUT OF PLACE BEING LIFTED BACK SLIPPED FROM THE POSITION IT WAS BEING LIFTED AN STRUCK THE MAN ON R ARM AND L SIDE OF FACE. R ARM BRUISED BY ELBOW.
EE WAS STRIKING A SCREEN RAIL RETAINING PIN WITH A HAMMER TOREMOVE A TIGHTENING WEDGE. UPON THE FIRST STRIKE THE WEDGE CAME OUT STRIKING HIS EYEBROW ABOVE THE RIGHT EYE JUST TO THE RIGHT OF TH E BRIDGE OF HIS NOSE. 3 SUTURES WERE REQUIRED.
