Wile roof bolting face area, a loose rock fell between rib and 1st installed roof bolt. Rock was 3' long X 2' wide X 1' thick. Rock hit canopy and operator moved into path of rock. Struck by rock in back of head and ankle.

Miner operator had backed continuous miner out to intersection to get a rock off miner. After rock had been removed, continuous miner was trammed into face to load a shuttle car. After shuttle car came in & got loaded, s/c operator started tramming toward feeder & as he got to rock in intersection, he hit it with car, flipping it into operator's cab. Size of rock 5'x 4'x 8"

A roof fall was discovered at the mouth of the intake portal at approximately 9:30 pm. The fall measured 10 to 12 feet high, 55 feet long and 18 feet wide.

Employee was replacing a cover over main breaker panel on the battery scoop. As he was replacing the cover he lost his footing causing him to drop the lid on his hand.

Employee was roof bolting and as he started drilling a hole, a piece of rock 18" long x 12" thick x 4"-5" thick fell striking him on the forearm. Shortly after that another piece of rock fell striking him on the shoulder.

Employee had backed cont miner out of #4 room to cut a corner off so that the shuttle car could get around easier. After he had cut the corner off, he backed the cont miner out by intersection waiting for roof bolter to bolt corner. While he was standing beside miner a piece of rock 5' long by 3' wide by 4-14" thick struck the employee.

Employee was carrying a seal block to the 2nd North #4 seal when he lost his footing, slipped & fell, landing on his rear on a timber. He re-aggravated the injury the next day when he was operating his shuttle car, hit a bump in the roadway & hit his head on the canopy of the shuttle car.

EMPLOYEE KNELT DOWN LEANING AGAINST THE RIB FILLING OUT HIS MAP, WHEN THE SECTION SCOOP OPERATOR CAME FROM ENTRY #1 TO ENTRY #2. WHEN THE SCOOP CAME THROUGH THE CROSSCUT THE OPERATOR TURNED THE CORNER HEADING OUTBY & THE BATTERIES ON THE SCOOP STRUCK THE EMPLOYEE IN THE CHEST & LEFT ANKLE.

EE WAS ROOF BOLTING AND AS HE WAS INSERTING THE RESIN CARTRIDGE INTO THE HOLE, A SLAB OF ROCK FELL. SLAB STRUCK EE IN THE SHOULDER AND BACK, PINNING HIM AGAINST THE CONTROLS OF THE ROOF BOLTER. THE ROCK MEASURED 1.5-15" THICK, 12-48" WIDE AND 9' LONG.

EE WAS ROOF BOLTING. HE HAD COMPLETED DRILLING A HOLE AND WAS ROTATING THE STEEL TO HELP GET IT OUT OF THE HOLE WHEN THE WRENCH CAME OFF OF THE STEEL AND OUT OF THE DRILL CHUCK, STRIKING THE EE ABOVE THE RIGHT EYE, CAUSING A LACERATION.

EE WAS SPREADING MORTAR MIX AT THE BASE OF A CONCRETE WALL. HE WAS MIXING THE MORTAR MIX WITH WATER & DURING THE PROCESS OF SPREADING & MIXING, THE MORTAR MIX GOT DOWN INSIDE HIS GLOVES CAUSING A BURNING SENSATION TO BOTH HANDS & TO HIS KNEES WHERE HE HAD BEEN KNEELING DOWN APPLYING THE MATERIAL.

EE STATED HE THOUGHT HIS SHOULDER POPPED BACK INTO PLACE & CONTINUED TO WORK THE REMAINDER OF HIS SHIFT & FOR THE NEXT 2 WEEKS HE WAS STILL EXPERIENCING SOME PAIN IN HIS SHOULDER.
