December 2, 2014 CO · Metal/Non-Metal bulldozer operator, universal operator, heavy equipment operator, operating engineer FALL OF FACE/RIB/PILLAR/SIDE/HIGHWALL
IGWT Mining, Inc. · Struck by falling object

EE was operating backhoe & stopped to grease machine. He placed bucket next to 15' highwall to grease. He said he was struck w/rock from HW on shoulder. MSHA rules & company site specific rules were not followed pertaining to highwalls. Training had included discussion of HW safety. Miner was never observed greasing machine next to HW before.
