INDIVIDUAL WAS WORKING AT END OF CONVEYOR AT THE END OF TUNNEL UNDER MOUND OF MATERIAL. WHILE VB BELT WAS RUNNING HR REACHED IN TO REMOVE ROCKS FROM BELT. HIS ARM BECAME CAUGHT AND WAS TORN FR OM HIS BODY BETWEEN ELBOW AND SHOULDER. HE HAD BEEN TOLD BY HIS SUPERVISOR TO SHUT THE BELT OFF BEFORE CLEANING ROCKS OFF THE UNDERNEATH PORTION. HE FAILED TO DO SO.
