WAS USING THE CHEATER BAR TO STRAIGHTEN THE SAFETY GATE ON ROCK SCALE. EE WAS PRYING DOWN ON THE BAR WHICH WAS UNDER ONE OF THE LATCHES THAT WAS BENT, THE LATCH BROKE OFF, CAUSING THE BAR TO S TRIKE EE ON THE FOREHEAD. EE CLAIMS HE WAS WEARING HIS HARD HAT, BUT WHEN HE TURNED TO MOVE AWAY, IT FLEW OFF.
