WORKER WAS STANDING ON THE COUNTERWEIGHT OF FRONT END LOADER HANGING THE KEY ON DIP STICK. WHEN HE ATTEMPTED TO STEP DOWN ONTO THE LADDER HIS FOOT WENT BETWEEN TWO RINGS AND HE FELL BACKWARD.
