The employee was off loading stone from the conveyor side of the Hyd. breaker and stacking on a pallet. EE's left 3rd finger became trapped between two large stones. EE was taken to Velocity Care and had x-rays and determined the end of the finger was broke.
Employee was stepping over water line pipe in route to place EE's lock on the disconnect for the Blade saw. EE caught EE's foot on the pipe causing EE to fall forward and hit EE's knee on the ground, EE's knee split open with about a 3" laceration. At the E.R. EE received 18 stitches.
Worker was stacking stone on pallet, stone slipped and crushed left ring finger tip between two stones. The finger tip was x-ray showing a fracture.

While stacking stone on a pallet a sharp piece broke off falling cutting the employee on left leg just below the knee. EE required 7 stitches at the local emergency room.

While employee was working discharge end of hydro split breaker, employee was putting stone on pallet when stone bounced back and caught right middle finger between stone and conveyor belt.
While palletizing stone the employee's finger was cut and broken when the stone the employee was moving slipped out of employee's grip causing the stone to pin employee's right pinky finger between the stone and back breaker belt.

While breaking stone on hydra split machine. A piece of stone broke off striking employee in EE's left leg (shin area) which cut EE's leg causing EE to go to ER and get 5 stitches.

Employee was operating an air drill. He was trying to reposition it in the stone when he felt pain in his lower back. As he tried to move it caused pain down his left leg.

Employee was placing stone onto breaker from pallet. Stone fell from table & caught little finger between table & stone.
Employee was breaking rock at VPI & quarry. Rock dropped off of breaker, breaking ring finger on left hand. He also had 9 stitches in finger.
Employee complained to co-worker on 5/7/12 that his shoulder hurt. Co-worker notified supervisor immediately. Employee claims that he strained his right shoulder sometime around 4/17/12 while pulling a rock to position it on the breaker. Employee was sent to doctor immediately. Employee has not returned to full duty at this time.

EE WHILE WORKING INSERT END OF NYD BREAKING MACHINE. WAS HELPING ANOTHER EE BREAK STONE. IN THE PROCESS HE WAS ROLLING A STONE UP TO BE SPLIT. IN DOING SO HE GOT HIS RIGHT THUMB CAUGHT BETWEEN STONE AND BREAKER TEETH. WHICH RESULTED IN HIS THUMB REQUIRING STITCHES AND ALSO BREAKING HIS THUMB.
WHILE CUTTING STONE EE WAS LIFTING STONE TO CUT AND FELT A PAIN IN HIS LOWER LEFT SIDE. HE THOUGHT HE PULLED A GROIN MUSCLE BUT FOUND OUT HE HADA HERNIA.
EMPLOYEE WHILE CUTTING STONE PICKED UP A SMALL STONE TO CUT AND TOOK A SHARP PAIN AND WENT NUMB ON HIS RIGHT SIDE.
WHILE CUTTING STONE, EMPLOYEE LIFTED A ROCK AND PULLED A MUSCLE IN HIS BACK.

EMPLOYEE WHILE PLUG DRILLING ROCK STEPPED OFF STONE TO PUT HOLES INSIDE OF ROCK, WHEN HE STEPPED OFF OF ROCK HE STEPPED ON A SMALL ROUND ROCK CAUSED HIM TO TWIST HIS ANKLE.
EE WAS TALKING TO BACKHOE OPERATOR & PUT HIS HAND ON BACK TIRE OF BACKHOE. THEN OPERATOR PULLED FORWARD NOT KNOWING EE'S HAND WAS STILL ON TIRE THIS RESULTED IN EE'S TWISTING ARM
EMPLOYEE WHILE REACHING INTO HIS STOCKPILE OF STONE PULLED ONE ROCK AROUND TO TAKE IT TO HIS BANKER TO CUT, AND ANOTHER ROLLED DOWN AND FELL ON HIS RIGHT FOREARM, PUT A CUT IN HIS ARM THAT REQ UIRED STITCHES.
