I NAME WAS TRYING TO DISLODGE A ROCK THAT WOULD NOT DROP INTO THE JAW CRUSHER AT THE SUB-BASE PLANT. HE WAS USING A LONG IRON BAR TO PRY THE ROCK LOOSE. THE ROCK BROKE LOOSE UNEXPECTEDLY WHI UNEXPECTEDLY WHILE I NAME HAD PRESSURE AGAINST IT AND THE BAR HIT I NAME ON THE INSIDE PART OF THE LEFT KNEE.

I NAME WAS DRIVING THE LOADER TOWARDS THE FUEL & OIL STORAGE AREA. ON THE WAY A HYDRAULIC OIL TUBE THAT CONTROLS THE BUCKET TILT SLIPPED OFF THE FITTING CREATING A MALFUNCTION OF THE BUCKET SI TICHES THE CUTTING EDGE OF THE BUCKET DUG INTO THE ROADWAY STOPPING THE LOADER.
