Employee legs been hurting from running 988B loader.Clinic-possible strain.Employee said from seat, seat replaced original was found in good working order.Continued working 10 weeks but due to reduction in work force laid off 10/22/10.Additional clinic visits determined torn right medial meniscus.

Employee was attempting to open a tube of silicon. The tube applicator has a device to puncture the seal on the tube, however, it was not long enough. Therefore, the employee took a welding rod and tried to puncture the seal on the tube with it. His first attempt was unsuccessful and on his second attempt he missed the tube and the welding rod punctured his left index finger.

Employee was removing bolts on loader cutting edge by keeping pressure on the head of the bolt with the palm of his right hand and loosening the nut with his other hand. The head of the bolt spun and cut the palm of his hand requiring stitches.
Stemming loaded holes with a shovel, felt a sharp burning pain in right shoulder.

EE WAS ATTEMPTING TO CLEAN SOME MATERIAL OFF OF A ROLLER ABOVE THE TAIL PULLEY. THE BUILT UP MATERIAL FELL FROM THE ROLLER AND ALLOWED THE BAR THE EE WAS USING TO GET CAUGHT IN THE TAIL PULLEY OF THE CONVEYOR, PINCHING THE LEFT INDEX FINGERBETWEEN THE CONVEYOR FRAME STRUCTURE AND THE BAR.
