While carrying buckets up the stairway in a yellow tub to replace buckets on EQ 250, the miner reached up to lift the tub over the railing. While turning and overextending during the lift, EE immediately felt a burning pain in EE's rib area and mid back area.

Miner was dropping the axle on a trailer and was preparing to cut the U-bolts when they bent forward at the waist and began experiencing extreme pain in lower back.
EE took off the top panel (lid) to EQ 215 screw auger. EE leaned the lid up against the wall. The lid fell and hit EE's shoulder.
The employee was closing the hood of the semi truck they were preparing to drive after completing their vehicle inspections.

Employee was stepping off of a forklift to dispose of an empty bag and slipped in mud and landed on EE's right elbow.
Miner was getting off the forklift when EE stepped onto uneven pavement and twisted EE's ankle.

Miner was cleaning on the mezzanine level when miner stepped on a piece of cardboard covering a hole and miner's foot sank in the hole twisting ankle.
Employee was running from one location to another in the plant and slipped and fell landing on their right wrist.
The miner indicated EE twisted EE's left knee when getting on and off the forklift. This employee normally works at a mine site in Glenrock, WY but was on temporary assignment at a site in Cuba, NM at the time of the injury.
The miner was attempting to step over a pile of snow and EE's right foot got caught on the pile of snow causing EE to lose balance and fall on left shoulder.

The miner was walking outside the plant inspecting ladders and didn't notice the trash barrel clamp against the wall and tripped over it.

Employee experienced pain in lower back on the right side after standing up following stacking pallets.

Employees were using a sledge hammer to break loose a nut/bolt on a piece of equipment. The hammer bounced back and smashed this employee's finger.
EE placed right hand underneath Conveyor 1 which was in operation at the time. EE's hand was pulled between two rollers resulting in abrasion and skin removal. There was no broken bones nor were the nerves damaged.

The employee was using a jack hammer to drive a sleeve off of a pipe. EE experienced some sharp pain in EE's shoulder but continued to work throughout the rest of the day. EE visited the emergency room later that day and learned EE had a slight dislocation of EE's shoulder which was put back into place at the ER.

Operators were cleaning up a spill, one using a skid steer the other shoveling into the bucket. At the time of the incident the operator stepped towards the skid steer and stepped into the skid steer causing left knee to buckle. The operator heard and felt a pop and felt an intense pain in left knee.
Employee was learning to use the 50lb bagging machine when EE took EE's foot on the foot pedal and the clamp closed pinching EE's hand.
Employee was working on cleaning a forklift after loading a bag of material into the machine. The employee went outside to clean off the fork lift after material had spilled on top of the unit. It had rained recently and the forklift was wet. The employee stepped on the right fork and slipped and fell. EE landed on hip, shoulder and elbow.

EE was working on the Rotex screener and while pulling up on the assembly the assembly gave way and ee hand was pinched between the screener assembly and the material inlet feed chute/pipe.
Employee was turning a pallet on a flatbed to unload it and finger tip was pinched between a metal plate and the pallet. Employee suffered a cut to left middle finger and required a few stitches.

Employee was removing the bottom cover over a belt on a conveyor during maintenance and it was heavier than expected. The employee was holding the cover at an awkward angle and his left shoulder popped holding the weight.

Employee was stepping down from truck and felt pain in his left shoulder as he stepped off the last step.
Employee began experiencing pain in her right shoulder, rotator cuff, triceps, deltoid and collar bone following the repetitive use of a mortar and pestle to grind material samples.
Employee was pushing on a bag on a scale to settle the material in the bag and felt a pain in his leg. After an exam by an urgent care doctor he was diagnosed with a pinched nerve. The employee was placed on restricted duty for 2 days.

The employee was carrying a hoe when she tripped over a block of wood frozen to the ground. She fell and landed on the handle of the hoe. The breath was knocked out of her and she became very tender and sore on her right side near her ribcage.

Employee was assisting with the installation of a die. The die moved into place and the employee's right finger was caught between the rollers that hold the die.
Employee was in the process of installing a battery in her radio when she dropped it on the floor and bent over to pick it up. She struck her face/eye on the corner of a tool box causing two lacerations.Employee was taken to a medical clinic and received stiches.

Employee was changing bearings on a crumbler roller when right hand was pinched/slammed and received approx. a 3" laceration to right palm which required stitches.
Employee was instructed to clean out crumblier. Employee took shovel off hook, turned, lost balance and began to fall. Because employee had not put safety grate over ladder hole, employee's left leg fell down ladder hole. Employee caught himself with right leg, but believes that he hyperextended right knee.
The mill was closed the day before. The coating tank was frozen when the plant opened Monday am. The plant mgr was trying to thaw out the coating tank by pointing the propane heater in its general direction. Plant mgr stood too close to heater and the back of his coveralls caught fire.

Employee was checking to see if covering (boot) was working properly on machinery. He stuck his hand underneath machinery and left index, middle and ring fingers were lacerated.
Employee was sweeping and cleaning on the 4th floor, plant manager was cleaning on the 3rd floor. When manager went to 4th floor, employee stated that she tripped and hurt her wrist. Employee was taken to local clinic, then transferred to hospital where they diagnosed her with a dislocated wrist.

Employee was sweeping area near truck drop (pit). He stepped on a 2" - 4" piece of humate, which caused him to fall one knee and twist his ankle.
