OPERATOR GOT OUT OF DRILL, BENT OVER TO SEE IF COAL HAD BEEN DRILLED THROUGH. WHEN HE RAISED UP HE HIT THE BACK OF HIS HEAD ON THE DRILL BIT WRENCH.

OPERATOR PUT HIMSELF IN THE WRONG POSITION TO CLEAN THE HIGHWALL AND PULLED A ROCK ONTO THE LOADWER HE WAS OPERATING. DAMAGE TO LEADER--WINDSHIELD WAS BROKEN, LIGHT BAR WAS BENT, FRONT FENDER WAS BENT.

AUGER OPERATOR STATED THAT THE SWING MAN WAS SWINGING STEEL OUT OF BELLY PAN ONTO THE GROUND TO START ANOTHER HOOK. VICTIM STATED THAT BUTTON ON HAND CONTROL STUCK AND CAUSED STEEL TO HIT HIM KNOCKING HIM ON OTHER STEEL SECTIONS LAYING ON THE GROUND AND HURT HIS BACK.
