While cutting a belt at the wash plant with a small razor knife the employee slipped resulting in cutting the top of his left hand. Went to local medical facility, received tetanus/diphtheria vaccination and four stitches. Did not loose time returned directly to work.

Employee was lying on back under 1" rock stacker at wash plant. While his feet were on the ground he pushed on conveyor and heard his knee pop.

Worker fell seven feet when his glove caught on a piece of metal that was being thrown off the side of the wash plant. Employee was changing a screen in the wash plant without his fall arrestor.

EE WAS CUTTING SOME OLD CONVEYOR BELTING WITH A UTILITY KNIFE & IT SLIPPED CUTTING HIM ON HIS LT. HAND BETWEEN THE INDEX FINGER AND THUMB. HE WAS TAKEN TO THE E.R. & HE RECEIVED STITCHES. EE SAID THE MATERIAL WAS ALREADY SCORED & HE WAS TRYING TOO QUICKLY. HE TOOK HIS GLOVE OFF SO HE COULD GRIP THE RUBBER MORE FIRMLY & THEN HE PULLED THE KNIFE TOWARD HIMSELF & SLIPPED.

EMPLOYEE WAS OPENING TRAILER DOOR TO GET PARTS. HE WAS TOLD THE DOOR SOMETIMES STICKS. HE WAS ON THE BOTTOM STEP, GAVE THE DOOR A HARD YANK AND LOST HIS BALANCE. HE FELL FORWARD, STRIKING HIS LEFT SHOULDER ON THE BOTTOM EDGE OF THE DOOR OPENING.

DRIVER'S HAND HAS CONTUSION, SWELLING, NUMBNESS, AND TWO WOUNDS 1/2 INCH IN LENGTH 2 INCHES APART. EMPLOYEE WAS WEARING HARD HAT, SAFETY GLASSES, GLOVES, AND SAFETY SHOES.
