Employee was replacing belts on crusher motor, when he lacerated the middle finger on his right hand.

Crusher was in final stage of reassembly & employee was attempting to move the material overflow chute back into position. He crawled on top of the unsecured chute to attach a Come-A-Long. When his full weight was on the chute, the chute moved from its unsecured resting position & fell to the ground 15' below. Employee landed on the chute as it hit the ground.

EE was working with the dredge operator securing the locking ring on a ball and ball connection on the 14" pipeline while rotating the ring EE elbow popped after three medical visits to examine ee elbow it was determined that his bicep tendon its separated and will require surgery to correct.

EMPLOYEE WAS IN THE PROCESS OF DRILLING TEST HOLES IN A GRAVEL DEPOINT. WHILE RAISING THE DRILL APARATUS, A STATIONARY GUIDE BOUND ON THE AUGER AND RAISE WITH IT. WHEN THE AUGER BECAME PERPEND IEULAR WITH THE BOOM THE GUIDE FELL DOWN THE AUGER AND INJURED THE EMPLOYEES RIGHT HAND.
