Customer truck driver entered facility and misaligned trailer box under fall safety rack so they could get on top and open box lids. Customer truck driver fell through gap between misaligned SafeRack halo and trailer box frame. Driver impacted the ground resulting in multiple non life threatening injuries. Emergency services were dispatched and tended to the individual.

Employee was entering the skid steer, and had used both hands to open the latch when thumbs overextended, employee went to the supervisor. Employee was sent to be seen by a DR. then was released with no restrictions and ok to return to work.

While opening Rail car gate the bar had slipped out of the cap stand and employee's thumb was between the bar and the ground.
EE was exiting the loader while hand was caught between the steps on the ladder, no pain or need to go to doctor, as weeks passed employee began to experience pain in wrist. EE was medically treated on 10-2-17 and released to go to work with restrictions.
Employee was unloading sand from a rail car using a conveyor, the bar that was placed in the capstan of the railcars slipped out causing employee to lean over belt - lost balance and fell onto belt. The result was employee going up the belt. Injury sustained was a broken right ankle and lacerations on face.
3 employees were involved, they were replacing the cutting edge on the loader. they had lower the bucket on the cutting edge there were bolts bound up, when they were to free up the edge EE and IE had there ends on the edge of the cutting edge causing to bruise there left hand equipment involved was. 972M Loader

Reportable hearing loss - standard threshold shift
Miner stepped off of a middles rung of a ladder thinking he was at lowest rung. The impact with the lower level injured his right hip and thigh.
Removing sand from malfunctioning truck tarp, spring loaded tarp arm struck miner in face when enough sand had been removed to release tarp.
