Ione Minerals · Caught in, under or between a moving and a stationary object

Employee jumped off forklift. Forklift was in neutral but still running. Employee said he failed to set the emergency brake. He reached across the forklift to grab something and accidentally hit the armature, putting the forklift into reverse and running over his foot.
