Assisting mechanic repairing haul truck. Loosening a bolt, slipped, and cut right index finger. Three stitches required.

Employee was handling a 4x4 dunnage and got a wood splinter in the palm of right hand. Employee pulled out splinter, but may not have gotten the entire splinter out. A week later employee went to doctor with what appears to be an infection the area.
We had a hose failure outside the back of the cab of our DM45 drill that broke out corner of cab window and sent some small glass fragments into the right cheek area and left forearm of the drill operator.

The individual was using a track drill on some boulders to make room for our new tunnel and while drilling was struck on the ankle/heel with a rock from drilling area.
Cone feed belt was jammed so EE removed the guard after locking out and EE dug the rocks out of the tail pulley. It freed the stuck belt up and it rolled backwards catching EE's pry bar and EE's right hand.
Individual was exiting cab of P&H crane and caught right foot in the process of climbing down and then fell to the ground.

Individual was a mechanic that was trying to loosen a nut on piece of equipment and EE's forefinger got between hammer and wrench causing injury.
Individual was fueling up D8 Dozer and finished fueling and was climbing down from dozer with fuel hose in hand and slipped and fell.
Employee was driving GMC fuel truck down haul road when cab locking mechanism failed and cab tilted forward and hit the ground while moving and driver used brakes to stop truck. Driver was wearing seatbelt.

Conditions were clear & cold w/below freezing temps @27 degrees. Subject reached overhead to pull slack out of come-a-long and his footing slipped and fell backwards with hard hat falling off. As come-a-long came unattached hitting subject in head.
Was working overhead with a grinder & welder. Debris from grinding on forehead and when individual stood up, it worked it's way into subject's eyes behind his safety glasses.

A EUCLID WATER TRUCK WAS BEING USED TO WATER THE HAUL ROAD LEADING TO THE UPPER PLANT WHEN IT STALLED, ROLLED BACKWARD INTO A BERM & OVERTURNED. THE DRIVER OF THE WATER TRUCK STATED HE BECAME VERY EXCITED & REMOVED, SEATBELT, JUMPED FROM THE TRUCK. HE SUSTAINED SERIOUS INJURIES TO SHOULDER, WRIST & ANKLE.
I WAS GOING DOWN THE HALLROUD AT THE END OF THE DAY CAME AROUND THE CORNER AND A DUMP TRUCK WAR COMEING UP ON ME SIDE OF THE ROAD TRIED TO GO AROUND THE OTHER SIDE AN DHIT THE FRONT OF THE TRU CK.
AUGER ON TRUCK CLOGGED UP. DRIVER UNCLOGGED IT AND WAS SWEEPING THE AN AWAY FROM THE DOOR WHEN THE AUGER STARTED UP. IT CAUGHT HIS GLOVE AND PULLED IT AND HIS HAND TOWARD THE AUGER. EMP GOT CU TS AND A PUNCTURE WOUND FROM THIS.
