THE SHIFT HAD JUST BEGUN AND THE EMPLOYEE HAD JUST ENTERED THE DRIFT AND WAS CROSSING THE CONVEYOR BELT, BELT WAS OFF, TO GET TO THE SIDE WITH MORE CLEARANCE IN HEIGHT ITS ONLY APPROX 36" HIG WHEN THE CONVEYOR BELT STARTED AND ENTANGLED HIS LEG IN A BELT STRUCTURE IDLER, HE MAY HAVE ACCIDENTLY PULLED ON THE PULLCORD
