Employee was bent over shoveling some spilled material when a rock either came off the screen or conveyer and hit him just below his clavicle on back. He first went home as it was quitting time. His back got sore so he went to hospital for x-ray. X-ray found he had a cracked bone. Off work at least a week and will return with restriction May 30 or 31.

JUMPED OFF LAST FEW RUNGS OF LADDER AND BRUISED HEEL WHEN HE LANDED ON A PRY BAR HE HAD PREVIOUSLY DROPPED THERE.
WIND BLEW A SMALL PIECE OF RUST INTO EYE. HE WAS NOT AWARE IT WAS EVEN THERE TIL THE FOLLOWING MORNING. HE HAD A DR. REMOVE THE PIECE OF RUST.

HE PLACED ARM ACROSS PIECE OF METAL HE HAD JUST CUT OFF WITH A CUTTING TORCH WHILE MAKING SOME REPAIRS. HE JUST FORGOT HE HAD NOT MOVED THE PIECE. HE BURNED ARM JUST BELOW ELBOW.
