OPERATOR EITHER BLACKED OUT OR WAS SOMEHOW KNOCKED OUT WHILE DRIVING THE HAUL TRUCK. THE TRUCK WENT OVER THE BERM AND DOWN THE EMBANKMENT. THE OPERATOR SUSTAINED A SCALP LACERATION AND 4 BROKE N RIBS WITH A POSSIBLE PUNCTURED LUNG. THE WEATHER WAS CLEAR, DRY AND DID NOT CONTRIBUTE TO THE ACCIDENT.
