The employee was removing a tray of #5 stone from the Gilson screener when he felt a sharp pain in his lower left back and leg.

The associate was cutting a piece of rubber for a rock shaker when the knife that he was using slipped and cut the back of his left hand. The cut was approximately 1/2". It does not appear that any damage was done beyond a soft tissue flesh wound.

Employee was helping change a screen, claims he hurt shoulder on 1/19/12. Employee states he told supervisor but supervisor does not recall being told. Employee stated on 2/2/12 that shoulder was hurting and wanted to seek medical attention. Originally there was no lost time but subsequently had surgery on 3/29/12.

DRIVER DUMPED LOAD OF SAND IN PLANT HOPPER. TRUCK WOULD NOT MOVE FORWARD, SO HE PUT IT IN "PARK" AND GOT OUT OF TRUCK. THE UNIVERSAL JOINT WAS BROKEN AND DRIVE SHAFT WAS LODGED AGAINST FRAME. EMPLOYEE KICKED SHAFT LOOSE AND TRUCK ROLLED ON HIS BODY, BREAKING HIS LEG, PELVIS AND CAUSING INTERNAL INJURIES. LEG HAS BEEN AMPUTATED.
