MattCo Materials, LLC · Caught in, under or between running or meshing objects

Guard on Cone was removed to replace skirt rubber, after employee adjusting conveyor while the guard was off for better visibility. Small amount of material was noticed inside the belt and tried to remove it with their crescent wrench, when the wrench became caught in the tail pulley, the employee's left arm was removed as a result of the entanglement within the tail pulley.
