Loose ribs

We had rope bolts, cribs in the area. Fault in roof.

Fall fell in intersection. They are putting up 8 foot rope bolts in intersection. They have gone to 6 foot, 12 foot rope bolts.

Shoveling under belt.

Hanging a curtain, a small piece of draw rock fell, striking between shoulder blades.

Roof fall in #5 Hdg, 4 Mains, spad #44 & 22, 70' in length. Had to move brattice line, cribbed heavy - old works.

Victim allegedly said rock hit him on top of his hard hat 40' inby spad then trammed pinner 80' to last open x-cut. Help hang pinner cable & got pinner out of way. Then he told co-worker what had happened. Co-worker escorted him to section foreman then escorted him outside, then took him to hospital. He refused an ambulance.

Unplanned roof fall primary intake between crosscut 4 & 5 on 5 belt impeded travel and obstructed ventilation.

A non injury roof fall was discovered at break #10 on the #4 belt in the #5 entry. It is approx. 30 ft long and 19 ft wide. The effected area has been dangered off with the intention to be cleaned up and to be rebolted.

Employee was lifting pulling a piece of belt structure when he felt a sharp pain in his neck. Pulled muscle.

Employee fell while exiting the man trip. His tool belt was caught on the steering wheel while he was stepping out. He fell backwards.

Employee pulled lower back muscle when he stooped over to replace his tracking reader battery.

Employee was welding and a piece of metal flew into his right eye. Pain and welding.

Employee tripped and fell into the ATRS while roof bolting.

Employyee cut right thigh with a belt splicing blade.

Employee was operating scoop when he hit a rock. He hurt his back when he bounced into the canopy.

Ee was stretching to lift cable high enough for bolter to go under and pulled muscles in his abdomen. Took two days off and returned to work with no problems.

Ee was working on a starting box on a scoop. As he lifted part of the apparatus, he strained his lower back. He is under doctor's care and still off work.

EE was climbing onto side of bolting machine to install glue and bolts, when he pulled a muscle. Ended being very minor with no long term effect at all. He only missed 8 work days.

Ee is a miner helper. He was moving miner cable from the roadway when he pulled muscles in his back. He only missed two days and came back to work with no problems.

Ee fell in a hole and bent the right hand backwards. Ended up with bones cracked in his fingers. Only missed one day, returned with no problems.

Ee was crossing the beltline when he slipped and fell to the ground, landing on his knee. After a doctor's visit, it ended up as only strained and bruised. He missed three shifts of work and returned with no problems.

Ee was pulling a 10 foot bolt when he strained his groin. Doctors have him off work. They don't know yet if it is a hernia or just a muscle strain.

Ee was striking a bolt with a hammer when a piece of metal went into his eye. The er could not get it out. The eye doctor got it out, no problem. Missed one day of work

Ee was taking apart a frozen water line. As he was pulling it apart it flew up and hit him in the nose. This broke a vessel and caused extensive bleeding. He had to have surgery to final take care of the problem.

Ee was shoveling when he pulled something in his back. It was only a pulled muscle. Ee went back to work, with no restrictions on 4-30-08

As EE was doing his normal routine roof bolting, he stepped on a rock, slipped off it and turned his ankle.

Ee was shoveling muck from a belt head when he experienced pain in his back.

A small piece of rock fell from roof and cut Ee's arm. A doctor put in two stitches and Ee went back to work, no problem.

A spring rod assembly came apart and a piece of it hit Ee on the ankle.

EE alleges that a co-worker slammed him to ground. The co-worker denies this.

Ee was working on the lights of a continous miner when a piece of rock fell from roof. The rock hit his arm causing him to jerk back. When he jerked, he pulled something in his back.

Ee was adding hydraulic oil to roof bolter when he felt a sharp pain in his back.

EE was lifting battery covers and felt something pull in his back. He tried to work it out, but had to go to doctor two weeks later.

Ee was working on brattices. He had removed part of one. Unknown to him, a block had stuck to the top until he got under it and it fell, hitting him in the back.

Ee slipped while roof bolting and twisted his knee.

Employee was putting up roof bolts. The bolt bent and mashed his thumb.

An unplanned, non injury roof fall was discovered by preshift mine examiner while making his checks and exam of mine. the fall occured between 4 45 pm on 9-1 and when it was discovered at 5:15 am on 9-2-04. the fall was at the 20th crosscut on #3 belt entry. it measured 0 to 8' thick, 35' long and 30' wide.

A ROOF FALL WAS DISCOVERED DURING THE WEEKLY EXAMINATION OF THE NO. 4 BELT FLIGHT MAINS. THE ROCK FALL HAD OCCURRED SOMETIME BETWEEN THE LAST EXAMINATION OF THIS AREA AND THIS PRESENT EXAMINAT ION. THE FALL MEASURED APPROX. 8 FT THICK, 30 FT LONG, AND 19 FT WIDE.

EE WAS STEPPING AWAY FROM A ROOF BOLTER AND HIT HIS KNEE ON THE MACHINE CAUSING BRUISING AND STIFFNESS.

EE IS A REPAIRMAN. A WRENCH SLIPPED WHILE WORKING ON A MINER CAUSING HIM TO HIT HIS RIGHT ARM & ELBOW. HE WENT TO CLOVERFORK CLINIC TO GET CHECKED OUT. ENDED UP ONLY A BAD BRUISE.

A NON-INJURY, ROOF FALL OCCURRED WHILE THE MINE EXAMINER WAS CONDUCTING HIS PRE-SHIFT EXAMINATION. THE ROOF FALL FELL IN THE INTERSECTION OF THE#2 ENTRY AT SPAD NO. 19+80. THE FALL MEASURED APPROX. 30'LONG X 20'WIDE X 10'THICK.

EE WAS UNLOADING STRAPS OFF SCOOP & STRAINED HIS BACK.

EE WAS CHECKING THE VOLTAGE ON A 1000 VOLT MINING MACHINE AND THE VOLT METER HE WAS CHECKING WITH MALFUNCTIONED CAUSING HIS HAND TO BE BURNED.

PRESHIFTING THE RETURN, DATING UP SEALS AND STEPPED ON A PIECE OF MINING BELT WITH LEFT FOOT. LEFT FOOT SLIPPED ON THE PIECE OF BELT, TWISTING LEFT KNEE AND HURTING LOWER BACK.

ROCK FALL IN THE INTERSECTION AT BREAK #37 IN THE #4 ENTRY. ROCK FALL WAS 25'LONG 20'WIDE AND 7'HIGH. TOP WAS LAMINATED SLATE BOLTED WITH 3/4 X 5' RESIN BOLTS. CRIBED AND DANGERED ROCK FALL OF F.

EE LEFT THE SECTION AND WENT OUTSIDE WITHOUT TELLING HIS SUPERVISOR. SUPERVISOR CALL OUTSIDE AND TOLD EE TO RETURN TO THE SECTION OR HE CONSIDERED HIM QUIT FOR WALKING OFF THE JOB. EE LEFT MIN E S8TE AND WENT HOME. AFTER HE GOT HOME, EE CALLED BACK TO THE MINES AND TOLD THE GUARD THAT HE HURT HIS BACK BY PULLING A PIECE OF BRATTICE CLOTH.

EMPLOYEE WAS HAULING A LOAD OF COAL FROM THE MINER TO FEEDER WHEN THE SHUTTLE CAR HIT THE RIB AND THREW EMPLOYEE FORWARD MAKING HIS RIGHT ELBOW HIT THE CORNER OF THE PANIC BAR.

MINER OPERATOR WAS TRYING TO PULL MINER CABLE AND WATER LINE OUT OF ROADWAY WITH ONE HAND WHILE HE HAD THE CONTROL BOX FOR THE 12-7 MINER IN HIS RIGHT HAND. HE TWISTED AROUND TRYING TO THROW C ABLE OUT OF ROAD AND PULLED MUSCLE IN HIS BACK.

HE WAS LETTING THE ATRS DOWN TO ADVANCE TO ANOTHER ROW OF BOLTS WHEN A KETTLE BOTTOM BROKE LOOSE FROMT HE TIP HITTING THE ATRS BREAKING THE KETTLE BOTTOM INTO 2 PIECES ONE PIECE HIT THE RIB AN D SLID DOWN THE RIB PINNING HIS LEG AGAINST THE ROOF BOLTER AND BREAKING IT IN 2 PLACES.

A PIECE OF DRAW ROCK SLID DOWN CANOPY ON ROOF BOLTER AND STRUCK EE ON LEFT ANKLE.

ROOF FALL AT XCUT NUMBER 35 IN THE #5 HEADING OF THE #1 MAINS. XCUT WAS BOLTED WITH 5; RESIN BOLTS AND TOP BROKE 2-3' ABOVE ANCHORAGE. THE FALL WAS 8'THICK, 24' LONG, 20' WIDE.

FEEDER FLIGHT CAME OUT OF THE FEEDER AND HIT EE ACROSS LEFT SHOULDER AND CHEST.

RIB WAS LOOSE. WHILE THEY WERE PUTTING IN SEALS, THE RIB ROLLED OFF AND BROKE ANKLE OF EE.

ROOF FALL IN FACE OF #3 ENTRY 20' WIDE 10'TO 12' HIGH 30' LONG ROOF IS SLATE TOP WITH PLACES IN IT THAT BROKEN UP.

EE WAS FIRED FOR MISSING WORK. HE WAS FIRED 4-23-01. ON 5-7 HE SAID HE GOT HURT IN THE MINES ON 3-29-01 THERE WAS NO WITNESSES TO THE ACCIDENT AND THE ACCIDENT WASN'T REPORTED TO ME UNTIL HE G OT FIRED FOR MISSING WORK.

HE WAS DRILLING A HOLE FOR A ROOF BOLT WHEN THE DRILL STEEL SWUNG AROUND AND HIT HIS LOWER ARM.

EE WAS WALKING BY RIB WHEN RIB FELL OFF AND STRUCK LEG.
