WHEN ACCIDENT OCCURRED TO JOHN ROARK MR. EADS PANICED AND JUMPED ON THE BELTLINE GOING IN WRONG DIRECTION AND DISLOCATED HIS LEFT SHOULDER.
WALKING UP ROADWAY & EMPLOYEE STEPPED ON A PIECE OF SLATE AND TWISTED HIS ANKLE.
THE ENDLOADER ENGINE STOPPED AND EMPLOYEE LOST HIS BRAKE AND JUMPED OFF EQUIPMENT.
