Checking Bobcat in doing preventative maintenance check. Slipped off rear bobcat tire. Landed off balanced and sprained right ankle.

Kasota Stone Fabricators Inc · Struck by falling object

990 loader operator was offloading finished blocks. Operator picked up the West block, employee approached the East block along its East end. East block shifted and 4x4 dunnage collapsed; the block fell & landed on employee's right foot. Operator used the loader to lift the block off of employee's foot, and EE "hobbled" away from the block to a safe spot. Crushed right foot.

Cleaning up tipping equipment. Air hose came loose and struck employee multiple times. Air hose struck employee to the head, neck, right shoulder, right ribs, and right wrist. Bruising and pain afterward.

Freeing the wire saw wire, which was pulling. Rock broke off from off the high wall and dropped on the middle of EE's back. Went into doctor and they advised EE to let the swelling go down and EE could return to work with restrictions. Expected light duty until 3/22/21.

Kasota Stone Fabricators Inc · Fall onto or against objects

Employee was filling up a large water tank using a smaller water tank. Employee was connecting the hoses and went on top of the tire of the smaller tank. Employee slipped and hit shin on a piece of support steel. Right shin bruise. No work restrictions initially. Employee went back to clinic on 9/18/20 and was placed on work restrictions. No lost time.
