WORKER DROPPED PRY BAR INTO CONE CRUSHER. HAD ANOTHER WORKER TURN OFF CRUSHER. AS CRUSHER WAS SLOWING TO A STOP, WORKER GRABBED END OF PRY BAR, CONE CAUGHT END OF BAR, WORKERS FINGERS PINCHE D BETWEEN END OF BAR AND FEED RING.

EMP WAS REMOVING A GUARD TO A SHIV (PULLEY). BELIEVED THERE TO BE 2 BOLTS HOLDING THE BOTTOM TOGETHER WHEN ONLY ONE BOLT WAS PRESENT. WHEN HE REMOVED ONE BOLT, GUARD DROPPED ONTO HIS FINGER. F INGER SMASHED FROM BOTTOM OF FINGERNAIL UP.
