West Plains Mining LLC · Caught in, under or between a moving and a stationary object

Laborer was manually rotating rotor to the area that was to be fixed. As it was being rotated his hands were placed on top and side of blow bar. As it was being rotated the left ring finger got pinched between blow bar and check plate resulting in taking partial tip of finger. The laborer was wearing leather gloves.

Labor was holding a piece of steel that was to be welded. Got it welded, laborer went to move and placed his hand on the spot that was just welded. Resulting in a burn to his right palm causing a blister.

Laborer was removing pump from a float pump, got in a bind, started pushing on it to free it up and it broke loose and came down and smashed his finger between frame work and pump. This happened @ the plant area.

Operator was digging into muck pile, started curling bucket back and felt rear end of loader getting light. Some loose rocks came over the bucket, down the boom into lower part of windows hitting the dash and breaking it loose. it came back into his knees. Operator got up and walked away.

Laborer was cutting metal with torch. While he was cutting, a spark came back and fell in behind his safety glasses, resulting in a corneal abrasion. There was no lost time.
