VICTIM HAD LEFT THE SURFACE MINE AREA WITH A LOAD OF COAL. HIS TRUCK WAS LATER DISCOVERED OVERTURNED ON THE HAUL ROAD WITH VICTIM UNDERNEATH. ITAPPEARED THAT VICTIM HAD LOST CONTROL OF THE VEH ICLE AND HAD JUMPED BECAUSE THE VEHICLE APPEAREDTO BE HEADED OVER AN EMBANKMENT.

INJURED WAS CHECKING COOLANT LEVEL IN THE EXCAVATOR WHEN HE REMOVED THE RADIATOR CAP TOO QUICKLY AND OUTIFREEZE SPLASHED IN HIS EYES. THE ENGINE WAS NOT HOT.
