Climbing up a ladder to a height of between 4-5'. When on the last step before stepping off of the ladder it folded and the EE fell approx. 4' to the ground injuring his back and twisted his ankle.

WE DID NOT KNOW UNTIL AFTER TALKING WITH THE INSURANCE CARRIER AND THE OFFICE OF MSHA THAT A REPORT HAD TO BE FILED.
