Keystone Aggregates Inc. · Over-exertion in pulling or pushing objects

Employee was leaving the premises at the end of his shift. He pulled on the chain used to lock the front yard gate. He heard and felt a "pop" in right arm. He pulled his tendon off of his right bicep. Please note that the date of this injury was also his last day of employment. He had previously given us his notice that he would be seeking employment elsewhere.

TWO EMPLOYEES WERE CHANGING A HAULBACK CABLE ON THE DRAGLINE. CABLE WAS LOOSENED & THE SLACK CABLE STARTED TO TWIST DUE TO RELEASE OF TENSION & TWISTED AROUND ONE EMPLOYEE'S RIGHT HAND. HIS HA ND & FINGERS WERE SMASHED - NO STITCHES, BREAKS, ETC., ONLY SWELLING & BRUISING.
