Employee was descending haul truck ladder steps. Employee miss judged placement of foot to ground and rolled left ankle as they stepped down to the ground causing a strain/sprain and abrasion of Left wrist palm side. X-Rays negative for fracture. Ground conditions dry, solid and smooth. Employee returned to work with restrictions.

Employee was unloading wooden pallets from F-150 truck bed. Employee stepped back with pallet in hands and stepped on another pallet causing EE to lose EE's balance and fall backwards catching self with EE's R hand causing a fracture to EE's R forearm requiring a cast.

Miner was rebuilding a guard on our VSI plant. They were using a grinder to cut a weld to remove a section of the guard to repair when the grinder they were using skipped of the weld and hit them in the thigh. It cut lower thigh about 3/4" deep by about 4cm in length. It require 5 stitches and the miner went back to work the same day. No impairment or future damage.

EE was working under a 769C haul truck on the driveline. EE went to get out from under it and when EE went to stand EE couldn't hold EE's own weight. EE was able to get to EE's service truck and call for help. EE was driven to the ER to be checked out. The doctor believes EE has potential nerve damage or it may be sciatica.

Putting new bucket tooth on loader and tooth fell from bucket onto right foot. Miner was wearing steel toe boots, but it just missed the steel toe part. The tooth fell from about 12" above the miner's foot. Large contusion and swelling. Time off to heal is the only requirement by the doctors so far.

Miner was strapping down a conveyor section on a flatbed trailer. As they were descending, foot slipped and they went to reach for a bar that was upright in one of the saddle strap holes. The bar slipped out of the hole and the miner fell to the ground below. The miner sustained impact injuries to right shoulder and hip. Miner was taken to the hospital for evaluation.

Operator driving between our 2 mines. As oncoming truck approached, EE slowed & moved to the side of road. Edge of road gave way under the wheels. Truck slid down short dropoff, turned on its side. Employee was wearing seat belt. Very minor damage to the truck. Employee had a sore neck. Was driven to a clinic. Dr performed no medical treatment. Recommended physical therapy.

Lab employee was helping slide a mechanical shaker back into place after being repaired. The shaker fell to one side and hit the top of the employee's foot as it fell to the floor. Metatarsal bone was broken.

Visitor truck driver fell and hit his head on the ground as he was loading his truck, which had broken down, onto a trailer for removal from the site. He grabbed part of the trailer as he stepped down for support, but his arm was weak and he fell to the ground, hitting the back of his head, which caused some bleeding. Received four staples in his head at the nearest ER.

Unknown. Employee found on ground; had hit his head on the ground. Appeared stunned and had no knowledge of why he fell. Could not remember falling. Had a bump on his head and a headache. Walked with help of a coworker to a vehicle for transport to hospital ER.

Injured person was unloading his flatbed trailer with assistance from pit employees and equipment. Two steel beams were left. One beam was picked up and forks caught second beam and trucker somehow got his little finger between beam and trailer when it came down. It was quickly lifted and first aid given. Driver had removed PPE gloves.
