ON HIS LAST DAY OF WORK EE'S TRUCK WAS SHUT DOWN AT 7:30 AM. HE OFFERED WORK TO FILL OUT THE DAY OF CLEANING UP THE SHOP YARD. A 966 LOADER AND CHAIN WAS PROVIDED FOR LOADING 75# DRILL ROLLERS ON A PALLET BUT HE DECIDED TO DO IT MANUALLY WITHOUT HELP AND PULLED MUSCLES IN HIS LOWER BACK.

EE WAS REMOVING HAMMER FROM DERRICK OF DM-45 DRILL. THE HAMMER STARTED TO SWING TOWARDS SERVICE TRUCK AND HE REACHED OUT TO STOP OT. WHEN HE CAUGHT IT THE HAMMER PULLED A MUSCLE IN HIS RT ARM.
EE WAS CLEANING THE WINDSHIELD ON A 992 CAT LOADER HE HAD SNOW ON THE BOTTOM OF HIS BOOTS. HE SLIPPED & FELL 10'3 5/8" CAUSING MULTIPLE TRAUMA TO HIS BODY.
EMPLOYEE WAS SLIDING A 50 GAL DRUM OF SOAP OFF OF PALLET ONTO FORKS OF A FORKLIFT. HE TWISTED TO THE RIGHT AND FELT HIS LOWER BACK ON RIGHT SIDE GIVE A SHARP PAIN.
