Employee was climbing down ladder from our Cat Water Truck at the end of the shift. As he touched his right foot to the ground (where there was a high spot of a rut due to previous trucks parking) the high spot gave way causing him to lose his balance twisting his right ankle.

DRIVER WAS HAULING LOAD FROM STRIP SITE (CLAY HARVEST) TO DUMP SITE. SPEED WAS APPROX 20 MPH. DRIVER STATED HE WAS HAVING A HARD TIME BREATHING. DRIVER BLACKED OUT, RAN THROUGH AND OVER BERM. TRAVELED APPROX 50' INTO FIELD BEFORE STOPPING.

EE WAS DRIVING A DUMP TRUCK & HIT A BUMP ON HAULROAD, STRIKING HIS ELBOW AGAINST DOOR.
INJURED WAS HANGING REBAR INSIDE DOME...IN MAN-BASKET OF P&H 50 TON CRANE. THE GROUND UNDER CRANES LEFT FRONT OUTRIGGER SUBSIDED AND THE CRANE LURCHED FORWARD, CAUSING BASKET TO EE UP AND DOWN . INJURED STRUCK HIS FACE ON STACK OF REBAR IN BASKET.
THE INJURED EE WAS HOLDING A REBAR DOWEL IN THE HORIZONTAL POSITION WHILE IT WAS BEING TIED DOWN. WHILE THIS PARTICULAR DOWEL WAS BEING SECURED, ANOTHER DOWEL WHICH HAD PREVIOUSLY BEEN TIED DO WN BROKE FREE FROM THE METAL WIRES SECURING IT. IT SPRANG UPWARD STRIKING EE IN THE MOUTH AND UPPER RIGHT CHEEK AREA OF HIS FACE. INJURED SUFFERED LACERATION TO UPPER LIP, BROKEN AND LOOSEN T
