Employees were removing empty oil drums from service truck. Injured employee was stepping down from bed onto bumper step. As he was stepping down form the bumper, his foot slipped and he fell to the ground landing on his hand.

EE WAS CLEANING ROCK BUILD UP FROM UNDER FEED TUBE. THE TUBE WAS RAISED UP TO CLEAN ROCK FROM TOP OF ADJUSTMENT RING COVER. THE TUBE WAS HOOKED TO BOOM TRUCK WITH 2 WAY SPREADER CHAINS. WHEN ANOTHER EE CLEARED A ROCK THE TUBE ROCKED FORWARD AND STRUCK EE'S LEG, PINNING HIS LEG BETWEEN FEED TUBE AND ADJUSTMENT RING COVER.

THE STOCK TRUCK WAS BACKING UNDER BINS TO LO AD OUT MATERIAL. THE EMPLOYEE STOOD UP THE SAME TIME THE TRUCK WAS ABOUT TO STOP PINNING HIM BETWEEN TRUCK AND CONVEYOR- CONCRETE FOUNDATION OF THE CRUSHER. EMPLOYEE SUFFERED BROKEN R SHOULDER
