EMPLOYEE WAS USING A GRINDER TO REMOVE BELT CLAMPS ON 7/26/97 AND A SMALL SPLINTER FLEW INTO HIS RIGHT EYE. ON 7/29 HE SOUGHT TREATMENT FOR REMOVAL OF THE SPLNTER. ALTHOUGHT THE EMPLOYEE REPOR TED THIS INJURY, THE SUPERINTENDENT WHO WAS ON VACATION WASN'T INFORMED WHEN HE RETURNED. UPON RECEIPT OF A MEDICAL BILL IN AUGUST AN INVESTIGATION WAS MADE & IT WAS DETERMINED A 7000-1 REQUI
