Employee was helping with changing impact crusher hammers. He got his left little finger smashed between the hammer wedges. His injury required stitches.

Employee was removing a screen bearing. A helper was tapping the bearing raise with a hammer to remove the raise from the screen shaft. A piece of metal came off the raise and hit the employee in the left elbow bend.

John Henry Quarries, LLC · Over-exertion in lifting objects

Alleged accident happened when miner moved rock to hold drawbar down on pump.He told other that he thought he hurt his back.he finished week out w/o incident.He called on Monday and said he was going to family dr and she put him off work until 12/27. we sent him to our occupational Dr on 12/14 and he immediately release him for lighduty as of 12/20 he has not reported back to work.
