After performing routine maintenance on the scalping screen, employee was exiting via a 9' fixed vertical ladder when lost footing and fell from the 3rd rung of the ladder to the platform below. Employee was transported to the emergency room and is currently losing time.

EE injured his right knee while jumping from a concrete block to the ground . He sought medical attention on 4/25/12 and was placed on restricted duty. This incident is being reporting within 10 days of employee seeking medical attention.

EE was climbing a ladder to a screen to perform maintenance and lost his grip and fell backwards. The fall resulted in lacerations to the employee's head and lower leg. The employee was placed on modified duty on October 13th and returned to full duty on October 17th.

While on a manlift, the employee shifted his weight and he had his middle finger right hand on the outside of the lift. As the lift moved slightly he smashed his finger between a beam and the manlift rail.

Employee complained that his lower back was hurting after operating the 980G and the pug mill. He was seen by a physician and medications prescribed and released to return to work.

The maintenance employee was working on Screen 2 (after it was raining) and slipped on surface causing a strain in the employee's back.

Two employee were unloading a pallet of wires. The hurt employee dropped the pallet causing it to hit his left wrist. Employee suffured left wrist sprain. This Accident was originally report online August 4, 2006; however, for some reason it never went through the system.
Employee suffered lower lumbar strain due to improper lifting/twisting of core samples. The samples weight is near 100(lbs). Employee was turning around core sample to read the label for inventory. The root cause of the injury were samples stacked to high. The contributing factors to the injury were improper lifting/twisting heavy objects. However, no damage or impairment.
Applying belt dressing to head pulley of pug mill conveyor, employee inserted arm behind guard and sleeve was caught by conveyor. Employee suffered arm dislocation, fracture, and abrasions before he was able to hit conveyor stop button.

Discharge conveyor stopped so he was turning rotor to look at the belt when his hand (right thumb)got cut by rotor blade.
While changing screens on the deister equipment, he was hammering wedges into screens and his elbow gave way.
