THE EMPLOYEE IS HAVING SURGERY & MISSING TIME FOR A SHOULDER INJURY FROM 1991 AT THE SAME TIME THE DOC LOOKED AT HIS FINGER BECAUSE HE COMPLAINED IT WAS HURTING HIM AT THIS TIME WE ARE NOT SUR E IF THIS IS RELATED TO THE SHOULDER INJURY OR NOT HE HAS BEEN OUT WITH THE SHULDER SINCE 06-08-94 & THE DOC SAID THAT HE MIGHT TREAT THE FINGER AT THE SAME TIME HE DID NOT REPORT THE FINGE

WHILE UNSCREWING A DIAPHRAM FROM A PUMP SHAFT WITH A WRENCH EMPLOYEE FELT SUDDEN PAIN IN RIGHT WRIST. AT THAT TIME HE SAW A DOCTOR WHO DIAGNOSED GANGLION CYST WITH THUMB TENDONITIS TO THE RIGH T WRIST WHICH THEY TRIED TO TREAT BUT ON JUNE 24, 1994 THE EMPLOYEE NEEDED SURGERY AND STARTED MISSING TIME. THE ACCIDENT WAS NOT REPORTED BECAUSE I MISSED THAT FACT.
