EE was walking under Conveyor PC-12 and slipped when they stepped on a rock. Their right knee jammed when their foot slipped. The EE was offered medical attention and refused at that time. The EE reportedly twisted their knee over the weekend when their foot got stuck in the mud. EE reported twisted knee to their Lead Person on Tuesday 3/24 requesting medical treatment.

Employee suffered sprain/stain injury when the return roller popped out of place as EE was inserting it into the bracket.
Employee suffered a neck strain when a sledge hammer head came off of a hammer, striking them on the hardhat. The hammer was being held by another worker as the injured miner was assisting loosening a bolt.

The employee was removing v-belts from an impact crusher. While removing the v-belts off the sheave, the whole sheave assembly fell off causing the v-belts to pull down weight of the sheave which struck the employee in the face. Initially this injury was first aid only; then the employee was referred to a dentist for further intervention.
While operating a dozer the employee felt a sharp pain in his right shoulder and shoulder blade. Initially this was first aid only. Employee terminated 11/06/07 due to work force reduction prior to being released to return to work full duty (right shoulder surgery performed 12/13/07).
