THE EE WAS USING AN IRON PIPE TO PRY LOOSE A LARGE STONE THAT WAS LODGED IN THE PRIMARY CRUSHER. THE IRON PIPE SLIPPED OFF THE ROCK AND STRUCK THE EE ON RIGHT SIDE OF HEAD THE TEMPLE AREA CAUS ING A CONCUSION OF THE SKULL INFECTED INNER EAR INFLAMATION IN BACK THE EYE.

THE EMPLOYEES WERE CHANGING OUT A 5 HP ELECTRIC MOTOR TO THE CONVEYOR. WHEN THE MOTOR WAS LOOSE, HE STOOPED OVER, INSTEAD OF SQUATTING, TO PICK UP THE MOTOR AND HURT HIS BACK. THE INJURY WAS C ONSIDERED MINOR DUE TO THE FACT THE INJURY OCCURRED ON SAT. MORNING AND WASN'T REPORTED UNTIL MONDAY MORNING.

THE EE WAS CLIMBING ONTO CRANE S TRACK TO REACH THE CAB. HIS FOOT SLIPPED OFF THE TRACK AND SPRAIN THE LEFT ANKLE. SOMEWHERE OTHER THAN THE EQUIPMENT THE EE HAD STEPPED IN GREASE WHICH WAS STI LL ON THE SOLE OF HIS SHOE.
THE AUTOMATIC WELDER WAS IN OPERATION BUILDING UP THE ROLLERS ON SECONDARY CRUSHER. EMPLOYEE CHECKED THE WELDING PROCESS, ON SEVERAL OCCASIONS, WITHOUT HIS HOOD IN PLACE.
EMPL WAS OPERATING PRIMARY CRUSHER WHEN ROCK BRIDGE OCCURRED. EMPL MOVED TO GRIZZLY AREA TO ATTAH LIFTING CHAIN WHILE ATTACHING CHAIN BRIDGE MOVED. ROCK APPROX 200 LBS HIT LEFT LEG. IMPACT CAU SED BY ROCK LEFT EMPL WITH ABRASION & BRUISE ONOUTSIDE OF LEFT LEG.
