Employee was carrying a driscoll pipe to install in a sump. The employee stated the ground was muddy, and as he was stepping onto a 2 step crossover, he felt himself slipping, and threw the pipe ahead of him onto the crossover. The pipe then bounced once it landed on the crossover, striking the employee in the shin.
On his eighth trip employee made a wide turn at a intersecting road because he seen a deer crossing. This resulted in right side of his truck driving into the base of the berm for 63 feet. At that point the truck had turned sideways and the left front tire caught the berm causing the truck to turn on its side. The driver unhooked his seat belt and left the cab.

In the process of unhooking a trailer mounted generator from behind a pick-up truck, the employee was attempting to loosen the coupler from the truck. While attempting to remove the coupler, the employee attempted to lift the tongue and felt a burning pain in his stomach. A bulge later appeared on the top of his stomach.

Worker was cutting a 3"X3"X1/4" angle iron using a 4 1/2" angle grinder with a 1/16" cutting disc attached. During the cutting process, the disc became bound up and the worker lost his grip on the grinder. The grinder twisted and the cutting disc came in contact with his right thumb, resulting in a laceration.

Employee was crawling out of a chute on top of a plank. As he was turning to come through the doorway of the chute he lost his balance. When the employee grabbed his lanyard with his left hand the jerking motion action pulled the left shoulder out of the socket.
The employee was using a bob cat to move clinker dust when the clinker dust blew up and into the employee's left eye getting around the safety glasses he was wearing.

Employee was walking from the bathroom to the lunchroom when he dropped a battery. As he went to retrieve the battery, which had rolled under a bin, a raccoon reached out and bit the employee.

While operating heavy equipment over the course of three days the employee experienced several events including running over potholes; large rocks and seat shift adjustments causing minor low back pain to escalate into severe low back pain.

The employee was removing a clamp out of a 12" drisco pipe at EV 5 51. When the last bolt was removed from the clamp, the pipe dropped down striking the employee in the chest. This caused the employee to fall to the ground landing on his left flank.
The employee was loading his truck while using 843-057 loader. The loader bounced over a hole in the ground causing the cab of the loader to jostle the oeprator causing pain in the low back.

ON 2ND VISIT TO DOCTORS 11-9-05 CHANGED TO REPORTABLE DUE TO MEDICATION PRESCRIBED.
