Employee stated he had an injury, that allegedly happened on 11/19/10. We were notified at 4:20 pm in a phone conversation on 12/07/2010. Employee has been off work since 11/23/10 on a non-work related health issue, results from some of his testing while in the hospital indicated 2 fractures in his vertebra, 1 old and 1 recent? Using a sledge hammer tapping grates into place?
Employee was assisting in setting a newly constructed loadout building onto the back of the boom truck. Rigging came loose on hoist causing the loadout building to shift causing the employee to fall to the ground. The employee fell approx. 54 inches resulting in a broken right wrist and a fractured little toe on the left foot.
Sample man was walking down muddy river bank when he slipped and fell bruising his right forearm.

EE along with two co-workers were in the process of loosening the winch cable rachet to loosen hard rigging to separate barges. EE & co-worker were pushing on rachet handle to get slack when rachet slipped striking employee. (This was reported to & filed with Dept of Labor Longshoreman because it occurred on the US waterway (river).
