Employee was climbing onto a Cat 777B End Dump when he heard a pop in his right knee. The accident happened on 12/13/2013 but the employee continued to work until 04/07/2014.

Employee was operating an end dump on the spoil dump when he went to dump the rock load the end dump traveled off the dump backwards overturning. The employee was taken to the hospital where he received 21 stitches total to the right forearm and left check area.

Hydraulic hose end blew off causing a flash fire. Employee received slight burns to arms. Treated and released for the burns. While leaving truck, employee struck his right knee and bruised his heel. Company doctor took employee off work until knee and foot healed.
Employee's seat bottomed out and hurt his back.
Driver was on top of his loaded trailer, moving loose coal. He fell off of the trailer, estimated height 12 feet. He landed on his back. Driver was not using PPF.
