EMPLOYEE WAS USING DRILL TO OBTAIN CORE SMAPLES WITH TUBES. EMPLOYEE PULLED TUBES OUT OF HOLE AND REMOVED CORES FROM CENTER OF TUBES. THE EMPLOYEE DID NOT ATTACH CHECKER COLLAR TO THE TUBES IN ORDER TO PREVENT TUBES FROM SLIDING BACK INTOHOLE. THE EMPLOYEE THEN HAD THE FINGER PINCHED WHEN THE TUBES SLIPPED.
