MW Sorter LLC · Over-exertion in lifting objects

The employee was helping lift replacement blow bars for the crusher repair. EE went to lift the blow bar and felt a funny pinch in their back. After resting for a minute, EE decided it best to go to the hospital for evaluation. The doctor released EE without treatment and told EE not to return to work for three days. All conditions were normal at the time.

The employee passed out on the job due to a personal medical reason. The employee was found by another employee. EE was woken up and seemed confused and shaky. 911 was called out of precaution and the employee was taken down to meet the ambulance. The employee was released to return to duty on the next set of shifts.

Fossil Creek Mines LLC · Fall down stairs

The employee slipped while walking down a set of stairs and landed on EE's back. EE felt good and that it was not necessary to go see a doctor. EE continued to work until 12/24/2022 and then felt bad enough that EE went to the doctor. The doctor told EE to take 10 days off and then return to light duty.

Fossil Creek Mines LLC · Fall from machine

It was a normal shift and weather conditions. The truck driver went to unstick the dump truck cover tarp and tripped and fell to the ground. EE braced the fall with hand and hurt wrist. We took EE to Urgent Care for X-ray and the results show that EE has two small bones in the hand that were fractured.
