HE WAS STANDING NEXT OT AN UNGUARDED JUNIVERSAL JOINT ON A LONG DRIVE SHAFT TO A GEAR BOX ON THE HEAD PULLY ON A STACKOUT DISCHARE CONVEYOR. APPARENTLY HIS COVERALLS CAME IN CONTACT WITH THE U NGUARDED U;NIVERSAL JOINT PULLING HIS BODY COMPLETELY AROUND THE DRIVE SHAFT & THE STEEL TOE BOOT ON LEFT FOOT GOT CAUGHT BETWEEN THE BRIVE SPROCKET & CHAIN CRUSHING THE STEEL TOE. 4TH TOE LF
