Miner was using a Milwaukee utility knife to cut a piece of rubber to use as a gasket for bindicator on classifying tank when the utility knife slipped out of the rubber cutting miner above top of left knee cap. Miner was not following proper cutting procedures by not cutting away from the body.

Miner was using an angle grinder on classifying tank discharge pipe to smooth out edges before installing rubber boot. Piece of metal got under safety glasses and into left eye. Miner was not wearing a face shield with the safety glasses per company policy.

Lycon Inc · Struck by falling object

Miner was placing a piece of tin on a small shed roof. The tin started to slide, miner went to stop the piece from sliding, edge of tin caused a laceration between index and middle finger on left hand. Miner received 2 sutures. Miner was given gloves to wear per company policy but decided at some point not to wear them. Cause of injury was not wearing protective gloves.
