May 11, 2026 CA · Metal/Non-Metal bulldozer operator, universal operator, heavy equipment operator, operating engineer SLIP OR FALL OF PERSON

While exiting the machine to take a break using the ladder handrail system the employee losing grip on a grease contaminated handrail resulted in a fall from the equipment approximately 3-4 feet to the quarry sand surface. This placed the operator off work and modified duty as of 05/11/2026 due to a lumbar strain/sprain.

operator doing pre-op not in accordance with standardized walkaround sequence for a CAT 745 Rock Truck causing the operator to fall from the truck platform causing lower back/buttocks injury placing the operator off work on modified duty
