EE WAS BACKING TRACTOR UP TO CONVEYOR TO MOVE IT. FOOT SLIPPED OFF CLUTCH AND HE BACKED UP MORE WENT UNDER CONVEYOR. PUSHED HIM FORWARD. HE BROKE STEERING WHEEL WITH FORCE. IN HOSPITAL OVERNIG HT HAS SEVERELY BRUISED BACK.

HE WAS WORKING AT THE CMI PLANT. WHEN HE BENT OVER AND PICKED UP A PLANK, HE EXPERIENCED LOW BACK STRAIN. HE DID NOT REPORT THE INJURY TO HIS SUPERVISOR UNTIL 12-27-92.
