EE WAS DIGGING OUT BLOCKS FROM BLOCK STACK. HE HAD REMOVED TWO BLOCKS FROM THE STACK, AND PARKED HIS OADER ABOUT 15 FEET AWAY FROM THE BLOCKS. HE THEN GOT OUT OF THE LOADER TO EXAMINE THE TWO BLOCKS WHICH WERE ABOUT 4 FEET APART. THE BACK BLOCK WAS UNSTABLE AND TIPPED FORWARD FATALLY CRUSHING HIM BETWEEN IT AND THE FRONT BLOCK.

EMPLOYEE WAS GUIDING STONE SAWING MACHINE ONTO TRACK WHEN CHAIN HOLDING SAW (BEING CONTROLLED BY CRANE) SLIPPED CATCHING RIGHT THUMB BETWEEN SAW AND CHAIN. EMPLOYEE LOST RIGHT THUMB NAIL AND R EQUIRED 5 STITCHES IN RIGHT THUMB. RELEASED TO RETURN TO WORK.
EMPLOYEE WAS HOLDING ON TO AIRBAG WHILE TURNING A CUT OF LIMESTONE. ADJACENT AIRBAG POPPED OUT OF CUT BEFORE DELATING. EMPLOYEE RAN, AIRBAG BOUNCED ON GROUND AND HIT EMPLOYEE ON RIGHT SIDE OF BODY.
