No known defaults of equipment which could have caused accident. Employee stated he was going to get off of the haul truck and just did not grab the step rails and fell backward. He said he felt pain in his back so we called an ambulance to make sure he was OK

Employee was holding a piece of steel to be driven into hole. Person swinging hammer hit the side of a bolt which deflected the aim of the hammer, which hit the employee's right thumb.
